Appointments, Cancellations & No Shows: Appointments are important to ensure adequate evaluation of mental health stability. The frequency of appointments varies depending upon the current presentation and can be more frequent or less frequent as time passes. You are responsible for attending appointments and agree to the following policy: We request you provide a 24-hour notice to cancel or reschedule an appointment if you cannot keep the scheduled appointment. If you cancel or reschedule more than three times, we may re-evaluate your needs, desires, and motivations for treatment. If you miss your appointment without notification (“No Show”), you will be charged the full self-pay appointment fee which is not covered by private insurance. Should you miss two (2) appointments without notification, you may be discharged from the practice.
Letters & Forms: Letters and forms completed for academic purposes will be completed at no charge. We do not complete long-term disability paperwork. Additional letters and forms will be charged a $25.00 fee and payment is required prior to pick-up. We request 72-hours to process any letter.
Legal Work: Legal work will be billed at a rate of $500.00 per hour. This includes letters to the court(s) and/or attorney(s). Any testimony will be a minimum of four (4) hours and includes any necessary time for preparation and transportation. The minimum of four (4) hours is in addition to transportation and preparation.
Emotional Support Animals: Emotional Support Animals are a companion animal that provides a benefit to an individual with a mental health disorder. In order to process an Emotional Support Animal letter, the individual must be an established patient of Wolf Pack Mind & Body and a fee of $250.00 will be charged. Not all individuals qualify to obtain an Emotional Support Animal.
Records Request: Records will be released only with an appropriate release of information completed by the patient and/or the legal guardian. Records are charged at a rate of $0.50 per page. Please allow up to ten (10) business days for processing records requests.
